To sign up, simply confirm your TARPEYO use and provide the names of a few healthcare providers. With your consent, PicnicHealth will compile your health history into a secure, regularly updated digital timeline that only you can access.

PicnicHealth will do all the work to gather your medical records once you sign up. With instant, secure connections to thousands of healthcare institutions, we’re usually able to find most of your records in minutes, and we’ll organize everything into an easy-to-use dashboard.
PERFORM Patient Registry combines and analyzes medical records
To sign up, provide some personal details and the names of a few healthcare providers. With your consent, PicnicHealth will compile your health history into a secure, regularly updated digital timeline that only you can access.
Records are yours to keep
We’ll continue to collect new records for the duration of the study. Any records collected are yours to keep forever, to use and share as you please.

The PERFORM Registry has closed enrollment for the study. We are grateful for the interest from the patient community.
We're sorry we weren't able to include you. Your interest in contributing to IgAN research means a great deal to us. You can visit [PicnicHealth's Study Page] at any time to browse all currently open opportunities.
No — your participation continues as planned. You will still receive updated records on your PicnicHealth timeline until the study ends.
Data collection activities are expected to conclude on or around [anticipated close date]. You'll receive an advance notice approximately one month before the registry closes, and a final communication once it has officially concluded.
Your PicnicHealth account remains yours at no cost. Even after the registry closes, you can continue to log in to access your personal health records, your timeline, health dashboard, care team, and smart health assistant. PicnicHealth will not collect new records on your behalf after the study ends unless you join another study or purchase a membership.
Any records collected during your participation are yours to keep, forever. You can access, download, and share them at any time from your PicnicHealth account. You can also continue to upload your own records to keep your account up to date or you can purchase a PicnicHealth subscription and we will continue collecting records for you.
The health data you shared as part of the PERFORM Registry will be used by the research team in accordance with the consent form you signed. If you have questions about how your data will be used, please reach out to us at hello@picnichealth.com.
