What is the Tepli-QUEST Study
The Tepli-QUEST Study is a real-world observational study in the United States. It follows people with Stage 2 T1D over time to better understand their experiences. Some participants have received an infusion of teplizumab and others have not, allowing researchers to understand a directly compared impact of teplizumab on Stage 2 T1D.
By allowing PicnicHealth to collect, digitize and de-identify your or your child’s medical records, you have the opportunity to participate in Stage 2 T1D research while staying in control of your health data.
Sanofi is sponsoring this study to better understand the real-world experiences of those affected by Stage 2 T1D. Your or your child’s de-identified data helps inform efforts to develop future treatments and improve care.


What to expect
Medical Record Collection
We’ll securely collect your medical records to understand your care and confirm eligibility. You can view them in your PicnicHealth account, along with tools to manage your care. They will be de-identified before they are shared for research.
Eligibility Verification
Once we get your records, we’ll check that you qualify for Tepli-QUEST — including a doctor-confirmed Stage 2 type 1 diabetes (T1D) diagnosis, age 8 or older, and U.S.-based care.
Paid Surveys
You’ll be asked to complete surveys about your experience with Stage 2 T1D, treatment, and overall well-being. These may happen every 6 months over 5 years.
Wearable Device Data
If you use a continuous glucose monitor or insulin pump, you can connect it to the PicnicHealth app to automatically share data.
Interviews
You may be invited to take part in occasional one-on-one interviews to share more about your journey.

The Tepli-QUEST Study combines and analyzes medical records, wearable data, and your survey responses
As part of this observational study, the ORBIT-CIDP Study team may analyze your medical records alongside those of other study participants to gain a clearer picture of how CIDP affects daily life. By combining data from many participants, the study can identify patterns and insights that may not be visible to any single physician.
To protect your privacy, all personal details that could identify you are removed through a process called de-identification, ensuring that researchers receive only the essential health information they need.

FAQs
You or your child may be eligible for the Tepli-QUEST Study if you:
- Have a confirmed diagnosis of Stage 2 type 1 diabetes (T1D)
- Have been prescribed or have been infused with teplizumab
- Are 8 or older at the time of enrollment
- Are 8 or older at the time of being prescribed teplizumab infusion
- Reside and receive care in the U.S.
The de-identified data gathered from the surveys, glucose monitors, insulin pumps, and medical records is helpful for researchers to understand real world disease and treatment experiences of people with Stage 2 type 1 diabetes (T1D).
Joining the Tepli-QUEST Study is completely optional. If you choose to participate, your medical care, including your Sanofi treatment will stay the same.
Yes, you can withdraw your consent at any time. This will not affect your medical care or future study participation. When you withdraw, PicnicHealth will stop collecting your records, but any data collected before will remain in the study.

Yes. You will be asked to complete online surveys as part of the study once you sign up and again every six months, which you will receive compensation for every time you complete a survey. It’s important you complete these surveys in a timely manner as they are intended to assess the impact of teplizumab on your condition.
Using a glucose monitoring device or insulin pump and sharing your device data is not strictly required, but it’s a key part of the study and you will receive compensation for taking part in these study activities. One of the main goals is to get a complete picture of the impact teplizumab has on disease progression.
You can still participate in the Tepli-QUEST Study with your medical records and survey data.

No, there is no cost to participate as you’ll receive PicnicHealth’s service free of charge as part of your participation in the Tepli-QUEST Study. If you would like to continue to have PicnicHealth collect your medical records, you may sign-up in the service as a customer at the end of the Tepli-QUEST Study or if you are not eligible for the Tepli-QUEST Study, you may opt to sign-up for a paid subscription.

Once you enroll and provide authorization, PicnicHealth begins by searching for your records through our connected network. If necessary, direct requests can also be sent to your healthcare providers to retrieve copies of your medical records. Some electronic records can be available within an hour, while records from individual providers may take a couple of weeks. Overall, this process can take a few weeks depending on how quickly providers respond. PicnicHealth will continuously update your timeline as new records are received and processed.
The Tepli-QUEST Study is not a clinical trial. A traditional clinical trial involves testing a specific treatment, like a drug or medical device. The Tepli-QUEST Study is a remote observational research study that you can complete from home. The Tepli-QUEST Study will only review de-identified survey answers, data from glucose monitors as well as insulin pumps, and your medical records produced during your visits to the doctor. In other words, there are no additional labs, tests, or appointments required for you to participate.
PicnicHealth will never share your records without your explicit consent. By signing up for this study, you're agreeing to share your de-identified data with Sanofi for the purpose of the Tepli-QUEST Study. No one outside PicnicHealth - including your doctor or insurance company - will be able to see your medical records unless you choose to share them by clicking "Send Records" at the top of the PicnicHealth timeline. You will be the only person who can access your medical records on PicnicHealth.

By gathering data from people using the medication in their daily lives, Sanofi can understand outcomes, side effects, and treatment patterns. This helps improve care, informs future research, and supports regulatory and safety monitoring efforts.
